There's an article in today's NY Times that provides some insight as to the disqualification rule.  Apparently here in the US the rule is that you're DQ'd only if you step three consecutive times on or over the line.  Int'l rules stipulate just one strike...

Spearmon was under the impression that, as is the case in the United States, runners are disqualified only after touching the line with three consecutive steps. International rules, though, maintain a one-and-done policy. Spearmon, who said he was not told about the rules by his coaches, asked some sprinting friends from Jamaica and Trinidad, and they told him they did not know about the rule, either.

“I’m the new dumb coach on the block with an athlete in the Olympics, and he gets disqualified because of a lane violation,” his father and coach, Wallace Spearmon Sr. said. “Everybody’s at fault here, but I’m his coach and his dad. That’s something I should have known.”

In the BBC commentary Michael Johnson was speculating the same thing, that she's probably carrying some sort of injury.
